A trip to the local food pantry may have changed one man’s life, with the help of a new app introduced last month by a Staten Island-based healthcare consortium.
The Social Determinant of Health App was developed by the Staten Island Performing Provider System (PPS) to connect residents directly with providers that can address specific environmental and lifestyle-related health and mental issues.
When the man first arrived at the pantry, he was interviewed by Megan Berardi, director of HealthConnect and a partner with PPS. She learned he suffered from mental illness, and from loneliness.
So using the app, she connected him with Venture house, a nonprofit that services adults with mental illness.
