SI CARES Patient Empowerment

 

Educating and empowering community members to better manage their health is a critical means to improving health outcomes on Staten Island. The SI CARES patient empowerment video series is a great resource developed through the DSRIP initiative by the Staten Island PPS. The objective is to optimize an individual’s ability to self-advocate and manage health care and to raise awareness of the availability of Care Managers/Health Coaches who can guide community members. The six 3-minute educational videos cover the following topics:

  • What’s a Care Manager or Health Coach?
  • Primary Care Doctor and Specialists
  • Hospital/Emergency Department
  • Pharmacy
  • Support System
  • Basic Needs – Food, Clothing, Housing, Transportation, and Financial Assistance

The primary target audience is Medicaid patients, however the entire Staten Island community will have access to the video series at different settings including hospitals, provider offices, online websites, social networking media sites, etc. Each SI PPS partner will play an important role in ensuring community members access these videos.

Hospitalized patients at high risk of readmission are receiving care management support for 30 days after discharge.
