After seeing success with Wellth’s Type 2 Diabetes program, Staten Island PPS is collaborating with Wellth again to launch a Gestational Diabetes program. This program will be launched in Richmond University Medical Center’s OB Clinic and Community Health Center of Richmond FQHC’s OB clinic. Under this program, a Medicaid OB patient with gestational diabetes can be enrolled for maximum 16 weeks. They can receive up to a $260 incentive for taking a fasting glucometer (measures glucose, or sugar, in the blood) reading and three post postprandial (after a meal) glucometer readings.
Wellth, an innovative smartphone based application, is designed to better engage and support hard-to-reach individuals. The app uses financial incentives delivered via Wellth’s mobile health platform. It can improve overall care plan adherence while delivering better health outcomes for women with gestational diabetes.
